I read through some feedback, there is a lot of differences -- not all feedback has a poll. Many feedback gives a positive or negative in the subject line.
It seems that the sticky on this forum is trying to indicate that feedback threads should be one thread per person being reviewed. It seems that the subject line should introduce the fact that it is a feedback thread, and give the name only. Then, the people giving the positive or negative feedback should include their comments such as positive or negative in the text of their post. When you have feedback for a person who already has a thread, then just reply to the thread with the new transaction and feedback. This way, a prospective buyer or seller can go to just one thread and see all the feedback for the other party.
Perhaps all feedback should have a subject such as (using myself as an example):
[Feedback]: OROutdoors
or
Feedback: OROutdoors
And perhaps all feedback should have a poll such as:
Please rate your transaction with OROutdoors (1=low, 5=high)
Then add 5 options to your poll, one option per line:

or
1 - Extremely dissatisfied, beware!
2 - Could have been better
3 - Just ok, neutral
4 - Good
5 - Excellent, well recommended
or the such
